Compass can carry out D4 HGV and PCV medicals at your business premises or at our clinic in Chorley.
We provide the following types of driver medical:

Compass driver medicals are carried out by an occupational health doctor and includes the required vision test.

HGV and PCV drivers need to complete the D4 medical as part of their first application for a Group 2 bus or lorry licence; HGV and PCV drivers also need to have a D4 medical at 45 years old and then every 5 years until they are 65 when they will need a medical completed annually.
Drivers who are under 70, hold a medically restricted car licence and held this licence before 1 January 1997 and wish to apply for C1 or C1/D1 entitlement (vehicles between 3500kg and 7500kg and/or a maximum of 16 passenger seats) will also require a D4 medical.

- Driving license
- List of current medication
- Glasses or contact lenses if you wear them
- Any hospital letters or details of any hospital consultants that are treating you.

The driver medical will take approximately 15-20 minutes.

- An eye test (if you wear glasses/ contact lenses, one test without and one test whilst you’re wearing them)
- Measuring blood pressure
- Testing of urine for diabetes (optional)
- Listing any medication you’re currently taking
- Asking about your previous medical history (these questions are on the form and are compulsory).

The DVLA D4 medical form is valid for 4 months from the date of the Doctor’s signature (ie date of D4 form completion), so you must send the paperwork in to DVLA within 4 months of your medical for it to be accepted.

The DVLA Guidelines state that if you are to drive a commercial vehicle you must be between 6 months and 3 years clear of mental breakdown. The time depends upon the severity of the condition e.g. Schizophrenia generally has a wait time of 3 years whereas anxiety attack would be 6 months.
